“I always call you first because I know you are willing to help me.”
~ J. Cellucci

“I like talking to you because it gives me the chance to talk about everything—my past, my present, my hopes, and my fears.”
~ M. Tyler
Serious illnesses such as heart disease, stroke, Alzheimer’s disease, cancer, Parkinson’s disease, liver and lung disease, and kidney failure slowly decrease an individual’s ability to live life to its fullest. Often, it can drain your energy and spirit leaving you unable to take care of day-to-day tasks.
The progression of each disease also takes its toll on family members and friends who find themselves in a caregiving situation with little or no community support. Assistance with resource management and support can be valuable at this point.
Transitions offers supportive counseling and community resources to people with serious illnesses. Non-medical support is provided through:
The Transitions Coordinators provide emotional comfort throughout the long journey of the illness and keep in touch with the individual and their family. They provide companionship and a listening ear when the burden of listening becomes too much for family and friends.
Your Transitions Coordinator will also work closely with community organizations to identify additional sources of help.
Transitions is sponsored by Delaware Hospice and offered free of charge within our service region.
One Transitions Coordinator said, “We often hear from clients and their caregivers that wish they had known about our services earlier in their illness. They are amazed that we can customize the level of support, depending on their needs, at no charge.”
