Delaware Hospice Center
Periodically, a person coping with a life-limiting illness requires specialized care beyond that which family members can provide in the home. When the need arises, the Delaware Hospice Center offers an alternative to hospital care, combining state-of-the-art technology with the comforts of home in a soothing, spacious and tranquil environment. Here, individuals receive expert, around-the-clock care, unlimited visiting hours and the support of a multi-disciplinary team. And, it’s a home away from home.
Who’s eligible for services at the Delaware Hospice Center? A person receiving our services may require a stay if the following occurs:
- Uncontrolled pain requiring assessment, medication adjustment or skilled nursing care
- A change in the intensity of symptoms requiring nursing intervention
- Respiratory distress
- The need for a higher level of medical attention or family support
- Family members require further education to provide care for a loved one
An individual’s physician continues to provide care for a patient and is supported by a multi-disciplinary team of staff members including:
- Delaware Hospice Medical Director: Consults with the patient’s primary physician, oversees the care plan and may also serve as the attending physician if necessary
- Registered Nurses: Manage the individual’s care plan and provides education to caregivers
- Certified Nursing Assistants: Provide personal care to patients and education to caregivers
- Social Workers: Provide emotional care, and support the needs of family members
- Chaplains: Offer spiritual direction and prayer
- Adult and Child Bereavement Counselors: Provide grief support
- Volunteers: Offer companionship and encouragement
