Comfort, Encouragement and Support Delivered at Home

Research indicates people prefer to spend the final days of their lives in the warmth of their own homes, surrounded by friends, family and familiarity. Delaware Hospice professionals work together with families and communities to coordinate pain and symptom management, and to provide emotional support to individuals coping with a life-limiting illness, right in the comfort of home.

A patient is eligible for hospice care when the prognosis is measured in months rather than years. It is critical for both the patient and his or her physician to align with the hospice philosophy, which provides comfort and peace to an individual once the focus shifts from finding a cure.

Delaware Hospice services are available in New Castle, Kent, and Sussex Counties in Delaware, and in southern Chester and Delaware Counties in Pennsylvania.

If you or a loved one would like to request services from Delaware Hospice, the following outlines our process in establishing service:

  • When initial contact with Delaware Hospice is made, we ask for just a few moments of your time to gather the most important preliminary information, including a description of the diagnosis and/or current medical status, along with the individual’s home address or living arrangements.

  • A Delaware Hospice representative quickly arranges for a personal visit from a member of our team, at the individual’s convenience.

  • A Delaware Hospice program representative visits with the individual to evaluate their medical and emotional needs, and conducts an interview to determine eligibility for services. The representative answers questions and reviews information regarding the range of options and resources available to individuals and their families.

  • Once the individual, and/or their family requests services, a registered nurse arranges to visit the family and completes the paperwork necessary for admission. The nurse provides patient care education, responds to needs for special equipment such as a hospital bed, oxygen, etc., and makes resources available to offer emotional, spiritual or volunteer support if the family chooses.
