After the Diagnosis: Living with a Serious Illness

Perhaps you’ve just been diagnosed with a life-limiting illness, such as heart disease, stroke, Alzheimer’s disease, dementia, cancer, Parkinson’s disease, liver and lung disease or kidney failure. You may feel overwhelmed with emotions or you may have many unanswered questions. We’re glad you’ve found us, and we want to introduce you to our Transitions program, which offers help and resources to individuals during the year or months preceding admission to Delaware Hospice.

Many people ask us to define the best time to connect with Delaware Hospice. Typically, we offer services to individuals when their time is measured in months instead of years, or when the focus shifts from finding the cure to comfort. You may find yourself coping with significant changes in managing your daily life. We can help.

When you contact Delaware Hospice, we ask you some preliminary questions to determine your diagnosis and prognosis. Then, we meet with you and determine your needs. Perhaps you’re looking for help in managing your household or with meal delivery; or perhaps you need legal counsel, spiritual guidance, financial assistance or help obtaining in-home medical supplies or equipment. We make references to local resources which provide the kind of help you need.

The Transitions Coordinator will maintain contact with you or a family member; and later, as the focus changes from treatment to comfort, will help you to connect with the services we offer at Delaware Hospice.
